  • Contact information to recieve invoices and SMS reminders for appointments.

  • Notes to support care planning and therapeutic progress​

This information is gathered with your consent and is used solely to collaboratively construct and maintain your personalised care plan. You can withdraw consent at any time, when safe and legally appropriate.​

All personal information gathered as part of your therapeutic care at Rise is treated with the utmost respect, confidentiality, and security.

Information will only be shared with your informed consent, except in the following circumstances:

  1. If there is a risk of serious harm to you or another person and failure to disclose would place someone at risk

  2. If information is subpoenaed by a court or required by law

  3. If you have given prior written permission for a report to be provided to another professional or agency (e.g. GP, support service, victims of crime)

  4. If you have agreed for specific information to be discussed with a nominated person (e.g. NDIS provider, support person or case worker)​

Your rights:

You have the right to:

  • You have the right to access the personal information held about you as part of your care at Rise. If you would like to request access, please email me at risecounellingandsocialwork@gmail.com. I will endeavour to respond to your request and provide the information within 30 days

  • Request corrections to inaccurate or outdated information

  • Withdraw your consent for holding your information (where safe and legally appropriate)
